    I understand the following first-aid supplies are kept in the nurse's office for standard first-aid treatment. (Please check the appropriate box to APPROVE your child to be treated with each)

    Acetaminophen 1-2, 325mg Tabs
    Ibuprofen 1-2, 200mg Tabs
    Cough drops
    Calamine lotion
    Petroleum-based jelly (Vaseline)
    Sterile eyewash
    Isopropyl 70% alcohol wipes
    Tums antacid 1-2 tabs (calcium carbonate 750 mg/tab)
    Pepto-Bismuth Subsalicylate 525mg or 15ml dose or 1-2 tabs (262mg)
    Saline wound care solution

    NOTE: For unexpected life-threatening emergencies ONLY, the school carries oral Benadryl and an Albuterol Inhaler, and an injectable epinephrine pen. Students prescribed to have epi-pens and inhalers must provide them to the school for their use.

    NOTE: Only essential medications will be administered to students during school hours, and only when no other option exists. Students may not bring medications to school. Parents/guardians must contact the School Nurse before bringing any medication to school.
